The following three items are due to the central office no later than-

- Health History Form This form must be completed in its entirety (do not leave anything blank), sign, and mail to:

- Email a digital headshot of yourself to photo@ccsa.cc saved inJPEG format named in the following way:
PROGRAM Last name, First initial example: LONDON/DUBLIN Doe, J

- Email a scan of your passport to passport@ccsa.cc saved in the following format:
PROGRAM PP Last name, first initial example: LONDON PP Doe, J
Note: If you do not have a valid passport, you should apply for one immediately. The U.S. State Department recommends, hence CCSA requires, passports be valid for a minimum of six months beyond the date you return to the U.S. If you are not a US Citizen, please review the information on Passport and Visa section of the website.
Requesting a Roommate-
On most CCSA programs, the program price is based on twin or, on occasion, multi-bedded accommodations. Program participants have the option to request a roommate(s), following the steps below:
Each individual must send an email to roommate@ccsa.cc requesting to room with a specific individual(s). The email should state in the subject line PROGAM Roommate Request, for example LONDON SUMMER Rommmate Request. Only requests confirmed by both parties (or all parties in the case of a request for a triple, if available) from separate email addresses from will be honored.
CCSA will make every effort to accommodate rooming requests. However, please be aware that in keeping with the study abroad housing policies of our member schools, no requests for opposite sex roommates (with exception of married couples) can be honored.
